Your First Clinic Visit

Your first visit helps us learn about you and your health. Here are some tips to help you get ready.

What to Bring

• A photo ID
• Your insurance card
• All your medicines (prescriptions, vitamins, creams, drops, etc.) in their bottles with labels

Before You Come

• Try to arrive 30 minutes early so we have time to get everything ready
• Bring a list of your questions or health concerns
• Fill out a health history form before your visit, if you can. If not, we’ll give you one at the clinic
• Fill out a medical records release form if needed

If You Need to Reschedule

We understand that plans can change. If you can’t come to your appointment, please call us at least one day before so we can find a new time for you.
