HESS HOME HEALTH

The primary goal of Hess Home Health is to provide home care services in an effective and efficient manner. This allows our patients to function at their optimal level while remaining in their familiar surroundings.

You don't have to give up the comforts of home when you are sick or in need of special help. Hess Home Health can help you stay in your home.

Anyone in the community may call to request services, including family, friends, ministers, social workers, physicians. The individual's physician must order home care services and approve a plan of care.

Who is Eligible?

Any person who has an unmet need for skilled nursing, occupational therapy, physical therapy, and/or health aide services, as determined by a registered nurse or therapist.

Any person who has approval of a physician for treatment.

Any person who lives in the covered service areas which include: Juneau, Sauk, Monroe, Vernon, Adams, and Jackson Counties.

Any person who is homebound.

Availability:

Home Health visits can be scheduled between 8:00 a.m. and 5:00 p.m., Monday - Friday and by pre-arrangement on weekends.
