Community Based Residential Facility

CBRF program goals:

To promote each residents individual level of independence.

To focus on retained abilities, rather than abilities lost.

To explore the possible cause behind a challenging behavior and provide planned intervention, if possible.

To provide a pleasant and friendly dining atmosphere to encourage good eating habits.

To provide meaningful activities programming to promote socialization, entertainment and exercise.

To maintain or improve the residents self esteem and feelings of self worth.

To provide a safe and comfortable environment.

 

             The Community Based Residential Facility (CBRF) houses 12 residents (8 private and 2 semi private rooms) and also specializes in Alzheimer’s/dementia care. 

Unlike the residents in the Woodland Wing, these residents do not require skilled nursing care, but do need 24-hour supervision and assistance.  Residents are assessed on an individual basis to determine their specific needs and the level of care needed.

 

Crest View Great Lakes is a home like environment that promotes socialization and interaction. Residents have access to their own kitchenette, dining/activity room, living room and laundry area as well as a secure patio area for walking, gardening and enjoying the outdoors.

The Importance of Family

 

 

Families participate in all aspects of the residents lives including  outings to the community and medical needs.

Families and friends are welcome to visit any time of the day or evening. Children and pets are also welcome with supervision. Families are encouraged to take their family member out for a meal, to church or to a family gathering.

Families are informed of any medical need or change large or small. When a resident needs to go to a doctor, families are asked to accompany them to their appointments.

Care Assessment, Care Planning and Implementation

Within 30 days after admission the DON, Social Worker, Activity Director and RCA will create an Individualized Service Plan (ISP) based on the residents needs. This plan will be reviewed with input from the RCA’s every six months. The ISP’s are reviewed at staff meetings so all staff are aware of the residents needs, approaches and goals.

Family members and guardians are invited to attend a Care Conference within 30 days of admission and every six month reviews thereafter. Family suggestions are elicited and added to the ISP. If the resident has a visiting nurse, that person is also invited to the Care Conference. The purpose is to create a plan that identifies the residents individual needs and establishes goals.

Therapeutic Activity Programming

The activity director and the RCA along with family input, access each resident’s interests and abilities and develop a service care plan that promotes their individual leisure interests.

Health Monitoring

Vitals and weight to be monitored once per month as necessary or more frequently upon physicians request. Information to be either faxed to physician or sent with resident upon routine visits.

Medication Distribution

The RCA are trained to distribute medications and document by the nurse. Medications are bubble packed through the pharmacy of choice.

Behavioral Interventions

RCA utilize interventions such as diversion, validation, reminiscing, therapeutic activities, and reassurance to diminish typical behaviors of anxiety and confusion. If staff interventions are not successful, the staff will consult with family regarding the need for appropriate medical or psychiatric intervention. Medications may be recommended and each residents primary care physician and psychiatrist reviews medications every three months (or more often) to determine side effects and the possibility of reducing medications. A resident may be discharged if there is imminent risk of serious harm to the health and safety of a resident or staff.

Spiritual Services

Area chaplains provide ecumenical services Tuesday, Wednesday and Thursday mornings as well as Sunday. The Mile Bluff Chaplain is available to provide spiritual and grief counseling to residents and their families. Residents individual ministers, priests or lay people are encouraged to visit and offer prayers.

Hygiene Services

Staff will implement a service plan defining the resident’s specific abilities and needs regarding hygiene and personal cares. Each resident will be encouraged to be as independent as possible and the RCA’s offer assistance as needed. Nail care is provided to each resident by staff or volunteers with the exception of diabetics. Baths or showers are always supervised by staff.

Beauty Shop Services

A beautician comes Tuesday and Thursday am to provide hair care to female residents. A barber visits monthly the second Monday of the month to provide hair care to male residents.

Nutritional Services

We offer three meals plus snacks daily. Meals are planned by a dietitian and all special diets ordered by the physician are honored. Nutritional supplements are offered when weight gain is desired.

Housekeeping and Laundry Services

Laundry is washed daily and returned to residents room’s. A housekeeper cleans the residents bath area daily and room weekly and the RCA provides daily monitoring of the room and cleaning services as needed.

Transport Services

Assistance with arranging transportation to outside appointments as necessary.

Money Management Services

Upon written request of the resident or appointed Financial Power of Attorney, Great Lakes CBRF will make available a separate, non interest bearing trust account for use by each resident. The account will hold up to $200.00 for resident to use for personal items, beauty shop appointments, lunch and movie outings. The account will be maintained by the Resident Trust Coordinator/Activity Director and funds will be available Monday through Friday from 8:00 a.m. to 4:00 p.m.. Residents should plan ahead for evening and weekend outings. Every resident will receive a monthly statement.
